ALiEM Cards is point-of-care reference library of narrowly focused, easily digestible cards for the practicing emergency physician or learner (formerly known as PV Cards). As of July 2017 led by the team of Dr. Jeremy Voros and Derek Sifford, we have rebranded these into “ALiEM Cards” and moved away from the index-card templated PDF files. Check it out.

Why transition to a new ALiEM Cards site?

We have always tried to keep the user and learner in mind. From a readability, design, and content delivery perspective, our index card look was becoming outdated.

  1. The limited text space negatively impacted our ability to easily convey the content in a clear fashion.
  2. The topics and keywords within each card were not searchable. You just had to know where things were.
  3. PDFs are often large-sized files and not easy to read on mobile devices.
  4. The new ALiEM Cards site incorporates the use of GitHub, which allows for better version control. We can more transparently update and incorporate crowdsourced feedback and peer review. This was not possible with the older PV Cards system.

What will we be stopping?

As part of this transition period, we are announcing the following:

  1. No more new content will be written in the PDF format.
  2. We are closing out the Evernote and Dropbox shared account options.

PV Card: Pediatric Ingestion Dose Thresholds for ED Referral

Clinical Toxicology has published guidelines for out-of-hospital management of 16 distinct overdoses and their dose thresholds, above which, pediatric patients should be referred to the Emergency Department for evaluation. Clinical Toxicology is the official journal of the American Academy of Clinical Toxicology (AACT, @AACTinfo), the American Association of Poison Control Centers (AAPCC, @AAPCC), and the European Association of Poisons Centres and Clinical Toxicologists (EAPCCT). There are two caveats to be aware of regarding these guidelines.

  1. They were developed between 2005 and 2007. New medications have been approved since that time and there may be more recent data available.
  2. As with any poisoning, dose is only one factor when determining disposition. Consideration should also be given to intent, underlying medical conditions, co-ingestion of other medications, presence of symptoms, and drug formulation.

PV Card: Local anesthetic toxicity calculations

Local Anesthetic LidocaineLocal anesthetics (LAs) are widely employed to achieve tissue infiltration, peripheral and regional anesthesia, and neuraxial blockades. Despite their well-established toxic dose limits, these agents continue to pose a substantial risk of morbidity and mortality due to local anesthetic toxicity and overdose.

PV Card: Emergency Drug Cards for Adults and Children

MedicationSyringeDrawRob Bryant, MD (@RobJBryant13), Amie Hatch, PharmD, BCPS (@Amie_EMPharmD), and Jeremy Bair, PharmD (@bairpharm) from Intermountain Healthcare in Utah have created and adopted a fantastic medication reference card which is used by physicians and nurses in the Emergency Department. The medications were chosen because they are often prone to dosing errors and require time-sensitive ordering. They generously offered to share this incredibly compact resource for free to the Emergency Medicine community as a PV card. If you see them, give them a high-five.

To use this sequential, four-question approach, if at any time you answer YES to the question, it is ventricular tachycardia.
